OPS Form Builder Manual
🧭 How to Use the OPS Form Builder
1) Open the Builder
Open the
OPS Network Form Creator
in any modern browser (Chrome, Edge, or Firefox).
The builder works offline — no server required.
2) Set Up Your Form
Form Title
— Main heading at the top of your form (bold by default).
Agency Name / Logo
— Optional header above the title.
Instructions
— Paragraph under the title that guides users.
Submission Email
— Required if you want to receive submissions via FormSubmit.co.
3) Manage Sections
Click
Add Section
to create a new section (e.g., “Contact Info”).
Use
Edit
to change a section’s title/description.
Use
Section ↑ / ↓
to reorder sections.
Use
Remove Active Section
to delete the selected section.
In the rendered form, each section shows a bold title with a separator line above it.
4) Add Fields
Choose a
Field Type
(text, email, number, date, time, text area, select, radio, checkbox, state, file).
Enter a clear
Label
(e.g., “Name”).
Select
Half Width
or
Full Width
for layout.
Optional:
Tooltip
,
Required
, and
Options
(for select/radio).
Each field offers: ✏️
Edit
, ↑ / ↓
move
, and 🗑
Delete
.
5) Preview & Generate
Click
Generate HTML Code
— the final code appears in the box below.
Copy Code
to clipboard,
Download
as an HTML file, or
Upload Code
to keep editing a saved form.
6) Embed or Publish
OPS Community Interface:
Paste the code into an “HTML/Code” block.
Submissions
go to the email you entered via FormSubmit.co (no backend needed).
7) Pro Tips
Keep section names short and clear.
Use Half Width for side-by-side layout where useful.
Download your form periodically as a backup.
Use Upload Code to resume editing any previously generated form.
